Dear Residents,

Please complete this form to submit your requests for your block schedule for the 2020-2021 academic year. We do our best to accommodate requests that impact your medical education, career, and significant personal life events. Day-to-day scheduling will be arranged in the future by the Chief Residents (e.g. so please do not request specific weekends off unless it is your wedding). Resident requests will be prioritized based on the order they are received.

The deadline for submission is Sunday, March 15, 2020.

That's a problem. Schedule your Step 3 immediately in coordination with the Chief Residents. You are required to take Step 3 before advancing to PGY-2, and your schedule requests will not be given priority.

Don't wait too long! If you have not heard from the Medical Board of California, check the status of each part of your application and give the Board a call if there is any discrepancy or uncertainty about your status. You must have your full medical license or postgraduate training license (PTL) before starting PGY-3, and your schedule requests will not be given priority if you do not have your license.

That's a problem. It may take 3-6 months to receive your license. Current PGY-1's cannot legally work past June 30, 2020 without a postgraduate training license. Current PGY-2's cannot work past June 23, 2020 without a full medical license or postgraduate training license. Please get your papers together and submit your license application immediately. Notify the Chief Residents immediately, and ask for help! Your schedule requests will not be given priority if you do not have your license.

And finally, let's discuss some specific scheduling requests

Remember, you will remain in the same Pod. Therefore your +1 Ambulatory weeks are set, and you can find out which weeks those are using the chart. Other rotations are generally 2-week blocks and should not overlap +1 Ambulatory weeks.

The Program will do its best to accommodate requests, and we will prioritize requests that impact your career path and completion of training. There will be a limit on the number of vacationers or electives in any given block, particularly during August January December. Vacation blocks requested in June and July are more likely to be assigned.

Please specify your preference for your first and second vacation blocks (each two weeks).
Specify any request for electives or other rotations (up to 3) and when you would prefer this to be scheduled.
