Schedule Request for Residents 2020-2021

Dear Residents,

Please complete this form to submit your requests for your block schedule for the 2020-2021 academic year.  We do our best to accommodate requests that impact your medical education, career, and significant personal life events.  Day-to-day scheduling will be arranged in the future by the Chief Residents (e.g. so please do not request specific weekends off unless it is your wedding).  Resident requests will be prioritized based on the order they are received.

The deadline for submission is Sunday, March 15, 2020.

    For assignment(s) to Ronald Reagan rotations, which do you prefer? (response required)

    (because you've never been assigned or were on service for less than 5 working days)

    And finally, let's discuss some specific scheduling requests

    Remember, you will remain in the same Pod. Therefore your +1 Ambulatory weeks are set, and you can find out which weeks those are using the chart. Other rotations are generally 2-week blocks and should not overlap +1 Ambulatory weeks.

    The Program will do its best to accommodate requests, and we will prioritize requests that impact your career path and completion of training. There will be a limit on the number of vacationers or electives in any given block, particularly during August January December. Vacation blocks requested in June and July are more likely to be assigned.

    Ambulatory Weeks for 2020-2021

    Instructions:

    1. Identify your +1 Ambulatory weeks using the chart.

    2. Please specify your preference for your first and second vacation blocks (each two weeks).

    3. Specify any request for electives or other rotations (up to 3) and when you would prefer this to be scheduled.

    4. At the bottom, prioritize your requests (priority 1 = highest priority).
