Schedule Requests for Residents 2025-2026

Dear Residents,

Please complete this form to submit your requests for your block schedule for the 2025-2026 academic year.  We do our best to accommodate requests that impact your medical education, career, and significant personal life events.  Day-to-day scheduling will be arranged in the future by the Chief Residents (e.g. so please do not request specific weekends off unless it is your wedding).  Resident requests will be prioritized based on the order they are received.

The deadline for submission is March 17, 2025.

Required responses are marked “(required).” If you have any trouble with this form, please email Ehab Salib and Dr. Ta.

    First, let's update your information

    Has your home address changed?(required)

Has your mobile phone number changed?(required)

Okay, let's get an update according to your level of training

What will be your PGY level in the 2025-2026 academic year?(required)

Have you completed Step 3? (required)

Will your ACLS certification be expiring by June 2026? (required)

Have you received your DEA License? (required)

    Are you considering subspecialty fellowship?

Are you planning to apply for fellowship this year? (required)

    Which subspecialty?

Is your OV Employee Health up to date?(required)

Have you been credentialed to work at the VA? (required)
All residents must be ready to rotate at the VA.

Now, let's consider some factors for scheduling

For assignment(s) to Ronald Reagan rotations, which do you prefer? (response required)

Are you interested in VA General Medicine Wards?
(Previously, General Medicine Wards was assigned to interns. This coming year, General Medicine at the West LA VA will be assigned to both interns and residents. The rotation includes a 1-resident-2-intern ward team structure plus Q5 overnight call for the resident)

Inpatient Geriatrics: PGY-3 residents will be assigned to both Santa Monica and West LA VA, two weeks at each site.

After this academic year, please check which rotations you STILL NEED to rotate on:
(because you've never been assigned or were on service for less than 5 working days)

Do you anticipate any personal life events?
(response optional but may help with planning)

When do you expect this? Please add any additional comments.

And finally, let's discuss some specific scheduling requests

Remember, you will remain in the same Pod. Therefore your +1 Ambulatory weeks are set, and you can find out which weeks those are using this chart. Other rotations are generally 2-week blocks and should not overlap +1 Ambulatory weeks.

Vacations and Electives: The Program will do its best to accommodate requests, and we will prioritize requests that impact your career path and completion of training. There will be a limit on the number of vacationers or electives in any given block, particularly November-January and April-May. Vacation preferences in June, July, and August are typically easy to accommodate. Elective time will be scheduled but the specific elective choice must be submitted for approval to the Chief Residents. Vacation and elective blocks should not overlap +1 Ambulatory weeks.
